ID Theft:
Forms provided by the ID Section or download ID Theft Form from FDLE’s web site. www.fdle.state.fl.us. Victims out of state can download ID Theft form from FDLE website and go to their local law enforcement agency to be printed. The printing agency must mail the forms to FDLE.
